Insure the individual involved in the incident or with the injury is safe. Mitigate any hazards and insure no other individuals were injured.
Once the hazard is mitigated and everyone is safe. Call the office immediately to inform them of the incident or injury.

PAPERWORK
&
DOCUMENTATION
If you or a GTC employee was injured, workers compensation forms will need to be filled out ASAP (optimally within 72 hours) and sent to the office (Controller). Each of these forms needs to be filled out and sent in.
These forms include the following:
Employee Accident Report - Must be filled out by the PERSON WHO WAS INJURED.
Declining Medical Treatment Form - Must be filled out by the PERSON WHO WAS INJURED. Use this form when someone informs you they were injured but that they do not want to seek medical treatment at the time.
Worker’s Compensation Incident Report - Must be filled out by the SUPERINTENDENT. Fill out all the information you know about the incident.
Management Accident Investigation Report - Must be filled out by the SUPERINTENDENT or SUPERVISOR.
